Interviewer instructions
Ask the respondent for the health/vaccination card of the eligible child. In some cases the respondent may not be willing to take time to look for a health card thinking you are in a hurry. Encourage the respondent to look for the card. It is critical to obtain written document of the child's immunization history; therefore be patient if the respondent needs to search for the card.

Once the vaccination card has been availed and you have seen it, code 1 for 'YES, CARD SEEN'. If the respondent says the child has a vaccination card but it is not shown to you because it is lost, someone else has it or it is not accessible during the interview period, record 'YES, NOT SEEN' for that child. If the respondent says the child has no card, record 'NO CARD'.
Take care to read the appropriate code and follow the correct skip pattern.
