I. Introduction of The Place
- The order of questions in Block I. "The introduction of The Place", in contrast to the questionnaire round I--that caused a lot of errors encountered-- are exchanged between the order of the name and the code to the provincial and district / city.
-The results of the first round found errors on Blocks I and II:
1. name of puskesmas, puskesmas's code, type of puskesmas and the number of other puskesmas underneath are not filled.
2. the name of the enumerator is not written, and the date of enumeration left empty.
3. the name of the respondent and his/her position is empty.
