Block 1: Identification of sample village/block having slum.

Block 5 : Particulars about slum including:
- Serial number of the slum, number of households in the slum, whether the slum is a declared one, year of declaration.
- Slum ownership, area type, slum waterlogging in the monsoon, availability of electricity, layout of the slum, type of structure, availability of roads, drinking water, sewerage, garbage disposal, distance from school and health centre and slum association.

Block 5: Item 25 - Whether there have been improvements in the condition of the slum over the last 5 years.
